off", since to pre-select one could be considered as prompting
for an answer whereas, in fact, it is essential that the person
completing the questionnaire gives their own honest opinion.
(That is, there is a distinction between the semantic meaning of
the page content and its intended purpose!)

It appears that you are missing a column. One that should be the
default checked.

Adding a Not Applicable (NA) column and having it the default
checked would also allow a user to change their choice back to NA.
