Tricky problem - how do I angle this in Photoshop?



If I've got a screen shot, or a box, whatever - how do I do this with it:

A screen shot/box, is angled the right side away, and made it look farther -
see, I can't even DESCRIBE it so I can't even Google for how to do this...!

I've got Photoshop 7.
