[vim] Keep getting stuck in Insert Mode

I keep running into this problem where I can't get out of Insert mode
and return to Normal mode. Hammering on <ESC> makes no difference.

Sometimes this happens when I first open a file. Other times it happens
in the middle of editing a file.

Restarting VIM usually fixed the problem, but sometimes not.

Any help would be greatly appreciated.
