We have also seen a high failure rate on the OC-48 modules, and after a lot
of testing we discovered the reason for it and it's easily corrected by
ensuring proper airflow through the system. If you are still around

We have been seeing circuit pack and intercard failures on OC48
transceivers on a lot of our OPTera 3500 network elements, which end up
being cleared by having the faulty card replaced (almost always an OC48
transceiver, PEC# NTN440BB or similar w/different wavelength). Most of
these were just turned up recently in many different areas of the
nation, which means they're not being turned up by the same people
(hopefully eliminating the theory of installation point of failure), in
many different environments. Has anyone else noticed a trend with these?
