A couple of questions .....
1. Does the recipient need Outlook2007 for this to work?
2. How does the data get back to the sender?
3. Does the sender do the dumping of data to Access? How?

I just read this thread and though instantly of Outlook. You can do
all sorts of surveys and such in there and dump it right back to
Access (directly). I also noticed that it has been suggested already.
Didnt know there was a wizard for it though - thats very helpful
indeed.
The only time I dont use Outlook for something like this (assuming
that Outlook is actually available to use of course) is if I have a
really massive data load to capture, such as shelf pricing from a
supermarket (each shelf, location, ean, price, store, date, time,
person). I use Excel and a custom form for that.
