I'd go for the comma.
Or maybe it should be "You are a sissy, boy crybaby!",
suggesting that "boy crybaby" is an appellation analogous to
"boy wonder"....r
How about "a sissy-boy" -- or would that work better with "cry-baby",
perhaps...
