In London we would use:
My Name
My Address
Tel: 0123 45678
...never "Phone".
...but we would *say*...
You can phone (or telephone) me whenever you like....
...never "tel".
My reaction, as well: "Tel." for written, "Phone" for spoken; never
one for the other.
