I realize this. If the observer cannot by sight alone tell if the
woman is nursing, then that's using a "cover-up". The cover-up can be
a draped cloth or an article of clothing that covers-up.
Just opening or lifting the clothing doesn't fall into the category of
using a cover-up (IMM). However, if the clothing is constructed to
provide a cover-up for the nursing mother, then a cover-up is being
used. The outside drape is commonly used because most clothing is not
designed to allow for the space required by the baby. I would think
that a woman who feels it necessary to buy a special blouse that is
voluminous enough would use an external cover-up if she isn't wearing
that blouse.
Do we have a signal there in your post that covering-up is the
convention you feel should be observed? "Proper clothing" would mean
proper for the situation.
