yes, it's all a question of chance a bingo in molecular biology and thus there are people with high A1c, high cholesterol, high blood pressure who are not overweight. People with the wrong genes, people who were exposed more to BPA, people who smoked too much, people who were in Vietnam together with agent orange etc, etc.
You can of course have an other theory. This theory worked for me : I lost the weight, did the exercise, got the T-shirt and for the moment have no med's and a much lower Insulin resistance. So for this correlation the reverse does work for a lot of patients : reduce the weight and get insulin sensitivity back. It works for so many people that governments are trying to restore the food quality and physical activity in a lot of countries.
Apart from epidemiological studies there are randomized control trials : weight loss by life style changes with the aid of a medical team, Rimonabant, Bariatric surgery all those mitigate the insulin resistance.
Then we have a number of mechanisms : adipose tissue gets infiltrated by macrophages if the fat store is too full leading to inflamation and insulin resistance. Excess food leads storage of fat in places where it should not be, leading to non-alcoholic fatty liver disease.
So a high BMI (which itself is a marker for fat in organs, fat stored at the wrong place ) leads to the metabolic syndrome. High HbA1c, bad lipids, high blood pressure and even cancer.
For me the evidence is strong enough to keep the weight off :) I don't have any more arguments, so I think we just have to disagree for the moment. If you found another lead or theory in the literature I hope you will let me know.

A few days ago we took an old friend to the party for his new grand daughter. He is in a wheel chair after a heavy stroke. His wife tries to take care of him, which is not too easy. We help them now and then with a transit bus that can accommodate a wheel chair. They are both in their seventies. He cannot talk much, his legs and one of is arms is not functioning. The contact, especially with the kids and the dogs cheered him up a lot.
Don't get a stroke ... :(
