Actually, she almost certainly IS saying precisely that. The concept
of tiered surgeries is still at least 3 years off here, probably
longer in Tracy's backwater!

Backwater ? The Welsh Assembly has found health to be one area where
it can take control and make a difference.
And it has done so with initiatives in many fields including Diabetes
care. It is going to champion innovations like GPwSI clinics precisely
because Wales is rural and sparsely populated in general and services
need to taken out into the community.
It might well be three years off where you are but Tier 2 GPwSI
clinics are in full swing here in the North West.

The GP with a special interest in
diabetes in my surgery has had zero extra training; he's simply stated
that he's got an interest in the surgery's literature.

And have you checked his credentials on the usual site ?
The PCT won't pay him extra and let him run specific diabetes clinics
unless he demonstrated GPwSI status.
