Note that the insulin deficiency comes "before" hyperglycemia has

Why are people diagnosed at the hyperglycemic level rather than the low
- dumb question?

Because the powers that be say so. It's one of my big bug-bears! To me, it
makes sense to treat a person before they start showing complications and
beta cell loss.
