In the program IIRC it was a blind man who identified the smell. That
would indicate that maybe people with all their senses may not have
noticed it.

I used to do ironing many many years ago for a man who said he had "insulin
dependant" diabetes. This was before I knew anything about blood glucose
problems in any way. His breath was terrible, it was fruity and it was
easily smelled from normal talking distance away. I also smelled the same on
my dentist, who by nature of the occupation was pretty much in your face
(before dentists started wearing masks).

A family friend around that same era who was in his 50's did a lot of
carpentry work in my house. I made a batch of scones for his first morning
tea and he declined. Said he was a diabetic taking insulin and ate 3 times a
day only, no snacks and bought his own food wherever he went. He accepted
just a cup of tea instead of my goodies :) I never once smelled anything
with him. In hindsight it probably meant the guy I ironed for wasn't in
great control??
