Attitude of being greatful that I'm not??? Couldn't have the attitude of
being greatful that I'm not, if I was. I would somehow make do with the pain
clinic, as would my mom, like my SIL has done. In fact, the clinic proved to
be the best thing for SIL. They were able to get her to comply, with things
my PCP could not, in order to receive treatment. As a result, she now has a
combination of meds that are working for her, and she was able to return to
work, working a 30 hr week job in the toursim industry. She is actually
taking less opiates, because she is taking the right ones. Plus, they were
able to adress issues causing her pain that my PCP was really not capable of
doing. Exactly what "attitude" would you expect me to have in this
situation? Or do you think I should somehow be mad because she is dropping
the majority? I'm more frustrated than mad, and it's directed at the DEA,
not at my PCP. Why be mad at someone who is still placing herself at risk by
retaining some of her patients for chronic pain treatment? And I happen to
be among the lucky chosen few, which she is under no obligation to do.
The point is, my PCP doesn't have to keep any; she has the option to refer
all to the clinic. She has chosen to cull her list down to long-term primary
care patients, and hopes this will lessen DEA scrutiny. It is possible she
has _had_ scrutiny, which led to this decision. People were referring others
to her, because there was no other local doctors willing to provide
palliative care, to the extent she would. The nearest pain clinic was an
hour away. Now that there is a clinic nearer, she is moving those patients
to there. It's possible Wellspan also has something to do with it, her
Osteopathic Primary Care clinic is affiliated with them, they provide her
lab supplies and services, and will be providing the PT at her location,
once renovations are complete. None of the other doctors here treat chronic
pain, all refer patients to the clinic. They do provide acute relief.
I get the impression from your seemingly terse reply that you have a problem
with my PCP's decision? This doctor has been a lifesaver for me, she is
everything you could want in a doctor; holistic, compassionate, thorough.
It'd be nice if the DEA wasn't micro-managing doctors, and putting good
doctors like mine in the situation of having to evaluate what she should
provide for primary care. After reading up about the latest DEA stuff
requiring PCP's to jump through more hoops in order to prescribe opiates; I
think she is making the best decision for the future of her clinic, and
seeing that those culled from palliative care are transtioned without loss
of meds, to the clinic. The clinic is better equpped to treat most of these
patients, than a PCP, and nearly all doctors in the Wellspan service area
send patients there.
Here I had hoped that someone might be happy for me, that my relationship
with my PCP is so good, that she chose to keep on providing my pain meds,
while sending most of her patients to the pain clinic, even though she could
choose not too. Usually this NG gets posts from either a) doctors unwilling
to assist pain patients or b) doctors dropping them, or c) not having anyone
at all available to prescribe pain meds. I also offered this as a good
reason to have a good relationship with a good PCP, when you have one; and
being transparent. Because this may prove to be favorable to others, as it
was for me. A doctor may make an exception for you, that (s)he wouldn't do
for other patients. It is really difficult to get any PCP to provide
continual pain meds; before her, I relied on the orthopedist that I was then
receiving care from. I no longer needed his continual care, so he released
me to her for care. She took over prescribing NSAIDS and opiates that he
formerly prescribed.
