Dear LC,
Thanks that looked pretty good. You'd have thought I'd have one of these
bookmarked already, but since they left out the part where you cut the dose
in half when changing medications "just to be safe", none of them are more
than ball park. But much easier than going to the PDR, converting everything
to morphine to figure out equivalents. That causes migraines.
