Comparing the US to Canada is shaky reasoning at best. Almost
all Canadians live in a narrow strip running East to West.
The environment and lifestyle is very uniform in Canada.
Take any problem, and the Canadian solution is much easier,
since there are 1/10 the number of variables.

In this case, there's a much larger percentage of the US
population that lives in backwater areas. We have hillbillies,
cajuns, closed pockets of empoverished Mexicans, etc. Even
if health care were free for them, how do you get it too them?
Bottom line: The South drags the US average down. Canada
doesn't have a "south". (Newfoundland doesn't count.)
