So whose should it be -- the doctors and pharmaceutical companies
and pacemaker manufacturers, all of them with a vested interest
in having as much done as possible without regard to value or
usefulness or pain or dignity?

My grandfather had a pacemaker fitted ten years ago. In his
case, it improved his health and quality of life and there was no
question it was worthwhile. But then the same doctors decided to
fit one to my gran as well -- no heart trouble, no medical
reasons we were ever able to find, only a doctor sitting in a
room with a worried and frail elderly lady telling her "it was a
good idea for your husband, madam, and I think you ought to have
one as well. Oh, and let's not bother any other doctor with a
second opinion, they're all frightfully busy."

Perhaps government is not the answer.
But neither is the answer to leave it in the hands of those more
interested in the bottomline than the line on the ECG.
