Medical code ?

I need to find the medical codes for fibromyalgia and for herniated
disks. Are these codes the same in each state? I just had to change
health insurance. Found out that my acupuncturist/massage therapist
gets very good cooperation from this company for her services. She
needs the medical codes for my diagnosis. I am going to a new doctor
in 2 weeks, and also wonder if they would give me the medical codes.
I am not changing clinics, my other PCP left the clinic.
