so ingenious."  Best example of that may be the guy who was crushed to
death by a robot (years ago) after climbing over a locked 10' high chain
link fence that surrounded the robot and its work area.

    If you want to keep people safe in a situation like this, then think
in terms of firing a lathe operator who'd even think of comming to work
in a long sleeve jacket.  That seems harsh; but compared to getting
shredded by a machine, losing one's job really is a "comparatively small
cost".

I kinda question your use of the phrase " repeatedly proven to be
totally impractical"- while I do agree that you can't stop fools from
killing themselves with machinery, and ensuring a safe dress code be
strongly enforced is fair enough- I don't see a safety mechanism on
the lathe that could have saved the poor mans life and I have to ask
why ? why would that be totally impractical ? we told the auto makers
to put seatbelts and safety glass in their cars- why can't we tell the
machine builders to make their product more safe and less dangerous
for the operator, and ensure shops provide reasonably safe equiptment
for it's employees ? what is a mans life worth compared to the parts
he has to make or the cost of even a simple rudementary safety device
that would have spared his in this instance ? certainly he is not the
first nor will he be the last killed in such a manner by a shop lathe,
but right now the same thing could happen in a dozen shops in north
america TODAY and unfortunately right now, there's nothing to prevent
it. I say there should be, as I'm sure his family would say and
perhaps you yourself, god forbid, were it to happen to your boy.
