"Next, you will fill out a questionnaire in which you are asked to provide your age, and if you have ever been convicted of a felony, theft, fraud, or a violent crime."
If you have an insurance agent friend, ask him/her what
your background check reveals.
If you're worried, pay the $35 bucks to have a PI
do the search. You'll find all kinds of stuff that
somebody with a name close to yours has done.
