It should just mean you wont hear a MIDI output, you will (should) hear
normal audio output just fine.
Like Nil said, what soundcard do you have? Soundblasters actually handle
MIDI internally so you should have a midi option and if not there is
probably a setting to enable it. If your using a "pro" soundcard things are
a touch more difficult.
Check your BIOS to see if you have MPU-401 (gameport)enabled, which is the
old gameport but also dealt with MIDI.
this thread and it's related threads may help solve your problem.

Also someone might be able to help you more here but something like
soundforge can handle the MIDI. I'm sure someone will point you in the
direction of a suitable program.
Also check out the cakewalk newsgroups, they are helpful.
Finally does Sonar LE have the Cakewalk TTS-1 synth? If so it's a softsynth
and you can use it to route MIDI through the Master output

I just installed Cakewalk Sonar LE Digital Audio Workstation. I
can't get started using it because a box pops up telling me to
select a MIDI Output from the list inside the box. The problem is
there is nothing in the list to select. No MIDI Output. I won't
be able to hear anything play until I bypass this hurdle. I've
got Microsoft Vista. Has anyone else installed this program and
encountered the same problem? What should I do now?
What version? I think Sonar 7 is the only one qualified to work on
Vista. If you have an older version, it might not work. Do I remember
right that Sonar LE is the simplified version that's sometimes included
with various soundcards and stuff? What sound card are you using?
Are you determinded to record MIDI? If not, you should be able to
continue on with no MIDI devices enabled and still be able to record
audio. My copy of Sonar 5 has a checkbox labled "Warn about no MIDI
devices" - when that's unchecked, the program won't bug me about that
at startup.
It's version 4.0.1. I don't know anything about MIDI...only that the
message in the box says I won't be able to hear anything without MIDI
Output. I've tried to record with no luck. And you are right. This
software came with the Samson C03U usb mike I bought. I'll check out the
newer version of the software. I still have Audacity to record with. The
mike works fine with it. And I'm very comfortable with Audacity. I just
thought I'd check out the software that came with the mike.
