I would think it an add-on causing this. I ran into something like this
when I was trying to do the seahorse quest pet leash was doing something
similar when setting the bait out.

hmmm... how could an addon affect the Blizzard phase of an area?



For me it was when I would set the bait my fluff pet would despawn and a few moments later Pet Leash would drop a new one out and bork the phasing. Try disabling your mods, your bar mod should be safe and try again.
