Isn't that a quest drop? You get a quest in a village to kill the HH,
go to SM, summon him, the book drops, you take it back and turn it in.

Not sure. I think I did get it my first run, where I used the yellow
quest, rather than the daily.

There are a lot of things that can falsely depress the drop rate that
Wowhead reports, so the 0.1% may not reflect reality.
