Now imagine the old raids like Molten Core, with 40 people! :-)
The first trick is to be able to see everyone's health bars, so that
you know who to heal. In a 5-man party you can see everyone's health bars
anyway, but there are addons that show you this information in a more
compact format that (in my opinion) is easier for the brain to process.
My preffered addon for this is Grid:

You can now see one square for each person in your party/raid, each square
representing their health bar. To target them just click on their square.
(There is a test version out with Pet support, which will be integrated
soon so you'll be able to see pets too)
As well as showing health, Grid can show a wide array of information about
each person in a compact (and initially cryptic) fashion. For example my
setup shows me, for each person:
Health Bar
Mana Bar
If they have an active Renew on them
If they have aggro (i.e. if a mob that is targetted is targetting them)
If they have a disease I can cure
If they have a magic effect I can dispel
If they are low on mana
I also have a Grid addon that indicates the suggested next 2 players
to heal, taking into account things like if they have aggro, if they're
squishy, etc. There are many other status indicators you can show too.
If you're not keen on the click-to-target business then I highly
recommend Clique:

With Clique you assign spells to different mouse buttons (and
combinations of shift/alt/ctrl-mousebuttons). For example, let's say
you make right mouse button = Flash Heal. Then all you need to do is
put the mouse cursor over either:
a) their normal WoW party icon frame
b) their Grid square
c) any other widget in your interface that represents them
and click the right mouse button, and you will cast Flash Heal on them.
No need to target them first, so you can keep your current target
on something you're interested in, like the main tank.
If your mouse isn't over a raid frame the button clicks act as normal.
I also love HealingEstimator:

Which shows you, in real time as you cast, what effect your heal will
have. You can see if you're going to massively overheal, or if your
heal won't be enough.
If you think you're never going to be in a party of more than 5,
then you might prefer something like this:

which provides icons that are attached to unit frames (including
pets) - click the icon to cast the associated spell on the player
its attached to.
I have never tried this addon, so I can't say how well it works.
