No, the instance is not reset after the last boss kill. If you enter
such an instance later, it is empty. You have to wait for the reset
and after that you can clear it again. Your raidinfo button shows you
beside the instance ID the time to the reset as well. By this it is
guarantied, that a person does a fully instance only once a week (for
40 man raids) or every 3 days.

This way, Blizzard can guarantee that raids keep doing MC until everybody is thoroughly sick of it just to get a pair of T2 pants (no T2 mage pants have dropped in something like 8 months of weekly Ragnaros kills...)

No they can guarantee that the bosses are not farmable. That's all what the raid-lockout system is there for. You can either have that or non-instanced raid bosses. Both ways have advantages and disadvantages.

As for loot problems, if you don't have free for all looting, switch raidleader and first person to enter (spawn) the instance. If you have ffa switch raidleader/inviter before anyone goes into the instance the first time. Loot in this game is assigned when mobs spawn, not when they die. Bosses spawn when the instance spawns and that happens when the first person enters it without prior raid id of said instance.

The rest is highly debated but I am absolutely convinced that the raidleader of the raidgroup does have an influence (not total control but an influence) of the loottables used for the instance. But I wont discuss this again, believe it or not, it's up to you.
