I've searched and can't find the answer to this. I connected my 360

One question - how are you trying to access the game?
The first time you play is different than every other time. I believe
the first time, it either launches itself after the install or
something like that. After that, though, you have to go into GTA IV
proper, then choose "New Game" and you'll have the option to choose
Lost and the Damned in there.
One of my friends was warning me of the confusing way to start the
game after the initial install the day after the expansion launched.
He said he wouldn't be surprised if a lot of people thought the game
wasn't working anymore.
Hopefully, this is the issue... if not, I don't think I have any other
ideas. :-\

I am able to play while my network cable is connected so I finished
the first mission and manually saved the game. When I load the game
(whether or not my network cable is connected) it tries to
automatically load that savegame but when it's disconnected I get a
message that it couldn't log me on (and do I want to check my
connection) then it goes on to load a new GTA4 game with Niko. If
during the Niko game I go to the main menu and choose new game I am
not presented with any lost and damned option, it just loads Niko
again. If I reconnect my network cable then it loads lost and damned
automatically with no problem.
I have checked around forums and even called XBox support, they had me
delete lost and damned from memory and re-download it which I did (it
took a few hours) but the problem persists.
I did have to replace my XBox due to the red ring of death. I wonder
if that is complicating the issue?
Here's the sequence:
* connected the cable, downloaded lost and damned, ran game fine
* disconnected the cable, ran the game fine for a bit until XBox died
with red ring of death
* sent XBox to MS (without the hard drive which I kept here), they
fixed it and returned it
* re-downloaded lost and damned as their instruction *** stated (for
any downloaded content) but won't work unless connected
* deleted from memory and re-downloaded lost and damned but problem
persists
I need to be able to run the game with the network cable disconnected
because my HD TV isn't anywhere close to the modem connecting to the
network. I can run a 100 foot network cable but that's not good for
the house. I can play lost and damned on my non-HD TV but it looks
very bad.
Hmm...
It really does sound like the license just didn't get transferred to
the replacement machine properly. If it did, the redownload should
have fixed this problem. One thing you can do is go to the Xbox.com
website and check out the license transfer tool.
If all of your licenses were truly transferred to the new console, the
tool should show 2 consoles with 0 licenses registered on one of them
and all of your licenses on the other (unless you've had more than one
RROD or more than just these two consoles).
I just did the license transfer tool recently when I bought a new
Arcade unit to replace my old one that RRODed 5 days after I got it
back from Microsoft. The licenses were all weird because some were
still on my machine BEFORE the repairs and some were on my old machine
after the repairs and some were on my new Arcade unit. I was able to
transfer ALL to my new machine with the tool.
You can only do the license transfer once per year, though, so just
use it first to see if there's any issue. If there is, you can do it
(but then you won't be able to do it again for a year, say if you buy
a new console within that time frame for one reason or another) if you
want.
