NYC vs. the rest of the country in an effort to isolate the demographic
responsible for spreading STDs...

How would doing that isolate the demographic responsible?

Let's say that the incidence of AIDS was indeed higher in NY. What
would that suggest is the "demographic responsible for spreading
STDs"?
