I started with cold water because I use a filter pitcher, not straight tap water.
Having "something" in the cup is not as easy as it seems. If the "something" is metallic you may get sparks. Otherwise (glass marbles) you have something that can spill out and splash, that you have to keep track of, etc. Much easier to just not overheat the water in the 1st place.
I use a wooden chopstick or small wooden spreader. I don't want chemical leakage from a plastic spoon or such. For my money, since the chance of severe burns and even maiming is so high if one miscalculates, it just seems smarter to a) use a teapot and b) put a wooden utensil (you could use ice cream pop type craft sticks) standing in the cup. We're not talking about a huge additional effort here, hardly worthy of lengthy debate. :-)
